Pale gums at home, or a low red blood cell count on a lab report, are two ways of reaching the same finding. Anemia in cats is a starting point, not a diagnosis, and the real work is finding out what caused it. The causes run from fleas to kidney disease to a problem in the bone marrow, and they are not equally serious.
The first question your vet answers is whether the bone marrow is responding. A regenerative anemia means the marrow is making new red cells quickly, which usually points to bleeding or to red cells being destroyed. A non-regenerative anemia means the marrow is not keeping up, which points instead to chronic disease or to the marrow itself. That one answer shapes most of what follows.
What Does Anemia on the Bloodwork Measure?
Anemia means your cat has fewer red blood cells, or less hemoglobin, than normal. Red cells carry oxygen, so fewer of them means less oxygen reaching the tissues.
Anemia is not a single disease but a shared endpoint. Blood loss, reduced production, and destruction of red cells all arrive at the same lab result.
Anemia is a finding to investigate, not a diagnosis in itself. The useful question is not how low the number is, but what pulled it down and how quickly.
On the report, the value to find is the packed cell volume, sometimes labeled hematocrit. A normal cat runs 25 to 45 percent, and anything under 25 percent is anemia [Cornell]. How far below normal matters, and so does how quickly your cat got there, because the signs track both severity and speed of onset [Merck].

Why Does Anemia Develop in Cats?
It helps to sort the causes into three groups: blood lost, red cells destroyed, and red cells not made.
In senior cats, chronic kidney disease is one of the most common causes. Failing kidneys make less erythropoietin, the hormone that tells the marrow to build red cells. Roughly 30 to 65 percent of cats with kidney disease become anemic, and the risk climbs with the stage of that disease [IRIS]. What Is the Difference Between Regenerative and Non-Regenerative Anemia?
This distinction is one of the most useful tools we have. Regenerative anemia means the marrow is answering, pushing out young red cells to replace what was lost, usually to bleeding or destruction.
Non-regenerative anemia means the bone marrow is not mounting an adequate response, which points more toward chronic disease, including kidney disease, or a problem with the marrow itself.
The test is a reticulocyte count, which counts those young cells. In cats, more than 50,000 aggregate reticulocytes per microliter means the anemia is regenerative [Merck]. Below that, the marrow is not keeping pace. This single number often narrows the list of causes before any other test is run.

When Is Anemia Something to Worry About?
Anemia deserves attention at any level. How urgent it is depends on how low the count is and how quickly it fell. The signs to watch for at home include:
- Pale or white gums
- Lethargy or weakness
- Reduced appetite
- Rapid breathing or a fast heart rate
- Reluctance to move or play
Any of these should prompt you to contact your veterinarian for a same-day appointment, not a wait-and-see week. Mild anemia found on a routine panel still needs an explanation.

One caveat on that table. The marrow needs three to five days to show a regenerative response [Merck]. A cat that started bleeding yesterday can look non-regenerative today, so an early count is worth repeating rather than trusting on day one.

What Happens Next After an Anemia Diagnosis?
Treatment follows the cause. That means parasite control and support for blood loss, targeted treatment for infections, immune-modulating therapy for immune-mediated cases, or supportive management alongside kidney treatment when that is the driver.
If a transfusion comes up, expect blood typing first. Cats are type A, B, or AB, and every type B cat carries antibodies against type A blood, so even a first mismatched transfusion can be fatal [ISFM]. There is no agreed packed cell volume that automatically triggers a transfusion; the decision rests on how the cat is doing, not on the number alone [ISFM].
